I am ordering the AVS4YOU Suite of programs for my multimedia computer lab at school. However, my county Board of Education in Clarksburg, WV, USA needs a copy of AVS4YOU's W-8 which has on it the Employer's ID #, Name, Address, whether the company is a corporation or a sole propietorship, and it is signed and dated. This is the fourth or fifth time we have tried to communicate with the company to get this information. IS there anyone out there who has a copy of this form??
