I am using AVS Video Converter to convert my BD discs to other video formats, after ripping them from AnyDVD HD. This process works on most discs but not all. When I open the index.bdmv file, AVS shows either only a few tracks, or a lot of individual tracks, none of them long enough to be a complete movie. Is this some kind of disc compatibility issues, enhanced security, or some other reasons? What should I do in this situation in order to convert a complete movie file?

On some discs you will need to add manualy the " m2ts" files from the Stream folder, as index.bdmv doesn't contain the full video information.
P.S. I'd recommend to add the files, which have been previously copied onto the hard drive.

How does one know how to arrange the correct m2ts files in the correct order? Also, if the index.bdmv file doesn't have the right information, it must be stored somewhere else as playing the disk doesn't seem to have any problems? Given that, I would expect the video converter should be able to figure it out as well, they just have to write the code.

The whole video info is stored according to the BD specs in the index.bdmv file.
The Video Converter does import the m2ts (video itself) and metadata info basing on the info in index.bdmv
It also will suggest you to choose whether all video tracks are imported.
All "well-built" BD should be imported while choosing index.bdmv
